Assisting with all activities of daily living such as feeding , hygiene , continence care ,dressing ,grooming ,transfers , oral care , bathing ,ambulation assistance , assist with medication and medication reminders observe client and environment and document and report any and all unsafe conditions complete and maintain related records and documentation such as communication books and progress notes
